So I couldn't find a valid boot disk anywhere (archive.org, mirrors.apple2.org and docs section). They did have the disks, but I couldn't find a bootable CP/M version. So I took the one labeled "Microsoft Premium Softcard IIe - CPM 2.26b (CPM files).dsk" and used my CFFA3000 to re-write the file resulting in the attached version that works for creating both 5.25" disk images and booting from the FloppyEmu.
